There is consensus that metformin is the first-line agent for patients with type 2 diabetes, but there is less consensus on what to do if metformin alone proves insufficient to control hyperglycemia. While I do individualize my diabetes treatment, I have found myself using the GLP-1 agonists as my preferred second-line agent (no hypos, some weight loss, pretty good reduction in hemoglobin A1c). I am curious to hear what other people use, and why?
