Electronic medical records (EMRs) have led to fundamental and important changes to quality improvement (QI) initiatives. I wonder if any experts could comment on specific initiatives at their instituition to reduce the number of medical errors? These could be preventative strategies or strategies to improve the response to a medical error. Furthermore, are you aware of any instances where EMRs have helped identify errors that otherwise may have gone unnoticed?
