The art of apology is obviously a challenging skill, but when the error is your own, the responsibility to apologize and deal with consequences fully lies with you. In the hospital, care is often transferred between practitioners, teams, and hospitals. Sometimes an error that the previous team made becomes apparent. Perhaps this error has even caused the medical indication for transfer to your team. How should you approach a situation when you know of an error that has caused harm to a patient, but was committed by a different practitioner or team? Thanks!
