A recent meta-analysis (JAMA Intern Med 2016; 176:958) showed that patients with chronic low back pain who are prescribed opioids do not experience a clinically important improvement in pain. How do we square this with patients’ reports that opioids are the only effective treatment for their pain? Should we be weaning even our “low-risk” patients who have been taking opioids for years?
