I'd like to take a moment to step back and put this study in context. Upon until the publication of this study we had no rigorous multicenter RCT to conclude whether or not Renal Replacement Therapy (RRT) is advised for patients in the ICU with an AKI.
As Dr. Dreyfuss pointed out, his center fully utilized RRT when patients were in need of treatment, but did not impulsively do so when the indications to do so were less clear. Meanwhile, other centers have had a more aggressive philosophy.
I would like everyone here to please share your INITIAL reaction to these results. Were you surprised by the findings? Did you question your approach? Did you have to reassess your action plan for any patients in your care?
