The new MACRA legislation uses a carrot-and-stick approach to modify health care provider payments and practice patterns. The carrot is the 5% Medicare bonus for physicians moving towards APMs such as ACOs and/or the bonus for high MIPS scores, while the stick is the risk of steep cuts (up to 9%) for those with low MIPS scores.
