These were some brilliant questions and comments! Thank you to the authors for the detailed and excellent responses. I was impressed by the paper, particularly by the large sample size, the advanced technical methods to measure and record BP readings, and the fact that the results are largely consistent with recommendations drawn from the SPRINT trial. It is also reassuring to note that baseline characteristics, as well as substantial event rates, were similar between both randomized groups within this study. While I agree that results may limit generalizability beyond this Chinese population, patients with low risks, and possibly even those aged 70 yrs and above, this study further reinforces the benefits of very close outpatient BP monitoring and early/aggressive intervention towards specific targets. I look forward to future studies that help clinicians address BP targets among the older, frail, dependent patients with dementia, who may already be on multiple anti-hypertensive medications.
