Referring to a medical student with effectively no social skills, Dr. Schwartzstein asks in his piece, "How did he make it to the fourth year of school?" and suggests that perhaps there is pressure or internal pressure not to fail a student. "They have worked so hard in all other regards, are in serious debt because of it, so am I justified in failing them?" seems to be the thinking. They don't want to "abandon" students, but have a duty to provide the public with empathetic physicians, so where does the answer lie?
