After watching your incredibly engaging interview with NEJM Group Journal Watch Editor Joe Elia, I wonder if a big part of this decision to test or not to test lies in the following:
(1) Do our patients have reliable access to good primary care so that we trust our patients when they say, "No doc, I have no medical problems"?
(2) Was there enough communication between the referring physician (arguably should be primary care, but many patients skip that step) and the surgeon? We all know that our system is incredibly disjointed and broken. Communication all the way around can be improved.
