Once the decision to commit to one specialty is made, it often becomes an identify. I have encountered multiple people who moved outside of the traditional pathway--whether switching from General Surgery to Internal Medicine after completion of residency or hoping to differentiate their practice and pursuing additional fellowship training midway through their career. Such decisions are often difficult, take courage, and require additional sacrifice. What factors would be most significant in such choices?
