While one of the major advantages of the PCP rounding is improved continuity of care and communication, there are many barriers (distance, time) which make this very difficult to achieve. Perhaps putting a carrot (small compensation) for communication between the hospitalist and PCP at both admission and discharge achieve similar results without? Of course it does not replace the personal touch of a PCP seeing a patient in the hospital but it's better than nothing.
