One of the barriers to the PCP becoming involved in a patient's admission to the hospital is lack of knowledge of the admission and the appropriate contact person. Certain hospital systems send an automatic email and page to the PCP when a patient is admitted to the ED. The next barrier then is figuring out how to get in touch with the hospitalist team. All of this is even harder when the patient is admitted outside of the PCP's network of hospitals. A faxed notification may come days after a patient is admitted. Are there any solutions to this problem?
