This was a very pragmatic "real world" approach to seeing how guideline driven serial procalcitonin measurement would affect clinicians' antibiotic prescription patterns in patients with suspected LRTI. In table S11 in the supplementary appendix, there was significant variability in clinician adherence to procalcitonin guided antibiotic prescription and I was wondering if the authors could comment on possible causes of this variation. For example, are centers with lower adherence referral centers with a higher proportion of complex or severely ill patients in whom antibiotics may be continued regardless of procalcitonin measurements?
